Important Change to Your Care Team

NP Leslie Conner’s care team is now set.Lorie Durazo and Sierra Bateman will be your designated care navigators. Please don’t hesitate to reach out to either of them with questions.
Additionally, Ian Simms will be assisting with third party lab ordering, records retrieval and medication reconciliation.
Communication remains unchanged, via the portal messages, by selecting “no preference” or any member of your team.

Out-of-Office & Holiday Notice

Dear Patients,

-Dr. Stuart Malcolm will be out beginning May 11th, 2026 for approximately four weeks for paternity leave. Dates are subject to change. Coverage from our other RTHM providers will be available for Dr. Malcolm’s patients during this time should you need assistance with your care.

If you send a message to your assigned provider during these times, please note that there may be a delay in response.

For any urgent needs, please contact your primary care provider, local urgent care, or emergency services as appropriate.
